Sinatra Top/Dress

The fabric I have selected is a cotton voile and the dress will need to be lined. Do you have instructions to line this dress? Also, if lined, I may not need the pattern piece #6...but in case I do, or for future reference, could you please tell me how to determine which piece #6 to use? There three different sizes availalbe. I am making a size 6 with D cup. Thanks so much!
